Registration and Membership Forms
Forms for Pathfinders and Staff, including medical forms, trip permission slips, safe driver forms and volunteer application forms. Basic Materials for Parents and Pathfinders: All Pathfinders and their Parents/Guardians must fill out a Pathfinder Membership Application, and a Medical Consent & Release Form. Clubs can make their own Membership Application form, or use the NAD form below. For most Texas and Central Texas Camporees, you will need additional copies of the Medical Consent & Release Form, each with an original signature. The Trip Permission Slip should be filled out for each event. You may want to have parents fill out one form without the signature and the event, and make multiple copies so for each event the parent will only need to re-fill-in the event and signature.
Basic Materials and Forms for Staff
Staff must fill out a Staff Volunteer Form, and read and acknowledge the Volunteer Code of Conduct. If staff will be driving Pathfinders to any event, they will also need to fill out the Safe Driver Pledge. Please note that we are now using Verified Volunteers for safety training and background checks. Participation is mandatory for ALL staff volunteers. Please visit http://www.verifiedvolunteers.com/ to register and take the course.
Safety and Security
Below are additional documents, forms and information to assist in training and planning. This includes the Adventist Risk Management (ARM) booklet on Pathfinder Safety, a field trip risk assessment checklist, an explanation of child protection and screening policies, and the ARM forms for filing insurance claims for vehicle damage or personal injury.

Uniform Information
The Pathfinder Class A (Full Dress) Uniform is an important part of the identity of Pathfinders. It symbolizes unity of mission and purpose, reminds Pathfinders of who and what they stand for, and presents a sharp and recognizable image for those observing the Pathfinders. Below are two files regarding proper placement of insignia. The first is the North American Division guidelines. Use this for the shoulder patches. The second is the Texas Conference guidelines for the placement of pins and insignia on the left chest pocket. This supersedes the NAD pocket guidelines. Shoulder Cords: Colors and Designations
(From NAD, Spring 2015): Shoulder cords are OPTIONAL, but IF they are worn, only ONE cord is worn, and that on the left shoulder. All cords (except the TLT cord) will consist of a braided main cord, two outside shoulder loops without ornamentation, and one gold pencil device. According to the NAD, if the conference adopts cords, ranks are represented by the following colors:
These NAD guidelines supersede the 2013 Texas Pathfinder Council vote on shoulder cords and colors. One possible exception is the use of the Purple cord for Pathfinder Bible Experience teams during their active Pathfinder year.
